- The range will vary based upon experience and licensure. Candidates who are registered nurses can anticipate a base salary between $70K-$80K. Non-RN candidates can anticipate $60K-$70K annual base compensation. $60K – $80K • Offers Bonus

The Role
We are seeking a dedicated and experienced professional to join us as PASA Case Manager. This role will report into the PASA Program Lead – Colorado. This is a Full-Time Hybrid position based in Denver, CO.
As the PASA Case Manager, you'll play a vital role in delivering person-centered care coordination for our PASA service lines in Colorado, including Community Connector, Homemaker Enhanced, Respite, and Supported Community Connector. With 2-3 years of hands-on experience in the PASA program, you'll manage patient intake, onboarding, caseloads, and ongoing support to ensure seamless service delivery on our established family CNA and IHSS foundation.
You'll handle day-to-day care coordination, collaborating with families, attendants, case managers, and stakeholders to support HCPF-compliant care plans and quality outcomes.
Key Responsibilities:
Manage patient intake and onboarding processes, including assessments, care plan development, and initial service setup.
Oversee a caseload of PASA participants, coordinating services, monitoring progress, and adjusting plans as needs evolve.
Provide ongoing patient support, including family check-ins, attendant guidance, incident reporting, and care continuity.
Ensure compliance with HCPF PASA regulations, operational memos, and documentation requirements like EVV and physician attestations.
Collaborate with the PASA Program Lead and cross-functional teams to resolve issues, optimize service delivery, and track utilization metrics.
Maintain accurate records in our EMR or similar systems, preparing reports on caseload status, outcomes, and quality improvements.
Build relationships with CMAs, CCBs, and referral partners to facilitate smooth transitions and program growth.
Participate in training, quality reviews, and continuous improvement initiatives for PASA services.
Key Requirements:
2-3 years of direct experience as a case manager or similar role specifically within Colorado's PASA program (CES/SLS Waivers).
Familiarity with PASA services such as Community Connector, Homemaker Enhanced, Respite, or Supported Community Connector.
Experience in care coordination, patient onboarding, caseload management, and Medicaid home health compliance.
Strong organizational skills with proficiency in Google Workspace, Salesforce, or equivalent tools for documentation and reporting.
Excellent communication and empathy, with the ability to support diverse families in a person-centered approach.
Ability to work independently in a fast-paced, hybrid environment while thriving in ambiguity.
Located in the Denver metro area; willing to travel across Colorado as needed.
Bachelor's degree in social work, nursing, healthcare administration, or related field (preferred).

Compensation Range: $60K - $80K
