The Cadence Health team is expanding and seeking a Patient Coordinator for our Advanced Primary Care Management (APCM) program to play a vital role in enhancing the patient experience and ensuring seamless care delivery. The Patient Coordinator will support Cadence RNs by coordinating patient appointments, mailing out care plans, sending care goal communications, and helping address social determinants of health. This role ensures patients stay connected to care, have access to needed resources, and feel supported throughout their health journey.

WHAT YOU’LL DO:

  • Call patients with care updates, reminders, and follow-ups on vaccines, diet, activity, and care plan check-ins.
  • Send care goal emails and patient communications, spending focused time each day ensuring timely follow-up.
  • Conduct proactive care coaching, including 60-day check-ins to ensure patients are staying engaged with their care.
  • Coordinate appointments with clinics on behalf of patients and advocate for their access to timely care.
  • Provide courteous and professional assistance to patients via phone, email, and chat, including administrative support and patient engagement activities.
  • Help patients address social determinants of health (e.g., SNAP benefits, transportation, access to healthy food) by finding resources and connecting them to support services.
  • Maintain accurate documentation of all interactions in Cadence systems in compliance with HIPAA and Cadence protocols.
  • Educate patients on Cadence’s technology and devices, empowering them to maximize usage.
  • Consistently complete assigned tasks and meet deadlines, ensuring patients receive reliable and responsive support.
  • Appropriately escalate patient concerns to the necessary care delivery teams, ensuring that complex issues are resolved efficiently.

WHAT YOU’LL NEED:

  • Active Certification as a Medical Assistant (CMA) from an accredited association (AAMA, AMT, NHA, or NCCT).
  • Prior experience in patient coordination, care management, or clinic support is preferred.
  • Comfort navigating digital platforms and patient care systems.
  • Ability to work independently while knowing when to seek guidance on sensitive issues.
  • Strong organizational skills with a focus on timely completion of tasks.
  • Excellent written, verbal, and interpersonal communication skills.
  • Proven trustworthiness and ability to handle sensitive patient information responsibly.
  • HIPAA-compliant home office setup with stable wifi and strong upload/download speeds.
  • Passion for patient-centered care and improving healthcare delivery through innovation.
